Eligibility: Residents of Hampton Roads with proof of income, proof of residency, a photo ID, Social Security Card, and a $55 retainer is required. Children enrolled in the school lunch program are automatically eligible for services. 
Eligibility Screenings: Monday through Friday: 8:00AM to 4:00PM 
Fees: All insurance plans within the Virginia network are accepted.  Copays and deductibles apply.   Sliding scale fees based on income are available for those without dental insurance. All dental visits are by appointment only.

Fees: Mandatory $10.00 non-refundable administrative fee must be paid prior to making dental appointment. $10.00 no-show fee for missed or late appointments, fee must be paid prior to making any other appointment
